Referral Pathway

Active in the Community (AITC) are delighted to announce that we will be spearheading the launch of a new referral pathway for residents of Buckinghamshire. Working in partnership with trusted local partners Age UK Buckinghamshire, Leap and Buckinghamshire Council, AITC will be looking to support 3000 residents in the next 3 years to become more physically active by embedding a referral pathway into the health and wellbeing eco-system. 

AITC will work with stakeholders across the county, from NHS/ICB colleagues to leisure providers and activity instructors, to support residents over the age of 50 and living with a long term health condition to increase their physical activity levels, improving residents’ physical and mental health and limit deconditioning and frailty.

Support on offer will include person-centered plans offering home and online workouts, support into community sessions and progressive programmes to encourage residents to move more in a way that works for them around their conditions, circumstance and local community assets. 

Chris Gregory, Head of Strategic Relationships at Leap, said of the new pathway “The Move Together Pathway is an exciting new service that will connect the health and social care sector with community movement and activity providers. Providing patients with dedicated behavior change support for residents living within our least active areas of Bucks.” 

Mark Russell, Chief Executive of Age UK Bucks, added “At Age UK Bucks we want to change how we all age for the better, so we can live healthier and more independent lives for longer. We are excited to be part of this programme working with AITC and Leap, because partnership will help us all build more movement into our lives, which, alongside good hydration, is crucial to keeping blood circulating effectively. Physical activity does not always mean running, swimming or going to the gym though. While these activities are really good for us, more movement can be built into our daily routine, for example sitting less, not avoiding stairs, or walking somewhere, if we are able, instead of taking lifts or transport.”

Level 3 Referral Specialist

We are looking for a Physical Activity Referral Specialist Level 3 (PARS) to be part of our Physical Activity Referral Service in Buckinghamshire. Working with GP practices across Aylesbury, Chesham and Wycombe – the project aims to support adults over the age of 50 with a long term health condition to access physical activity opportunities. The successful candidate will conduct initial assessments, plan, prescribe, implement and monitor a suitable exercise programme for individuals referred through the pathway.
