Referral Pathway
Supporting you to move more, feel better, and stay independent!
Move Together Buckinghamshire is a free programme designed to help residents aged 50+ living with a long-term health condition to be more active.
Funded by Buckinghamshire Council’s Public Health Team and delivered by Active in the Community (AITC), this three-year initiative is all about helping people move more in a way that suits them, with professional support every step of the way.
What's Included?
If you’re eligible, you’ll receive up to 12 months of support, including:
- A personalised 12-week physical activity plan, tailored to your needs and abilities
- Help accessing local, affordable activity sessions
- Ongoing encouragement to build confidence and stay motivated

Eligibility Criteria:
- Residents who are 50+ years old.
- Currently doing less than 30 minutes of activity a day.
- Motivated to increase activity levels
- Currently living with a long-term health condition (see approved conditions listed).
- Registered at one of the 17 listed GP practices or live in one of the Opportunity Bucks wards (mentioned above).

Meet Our Referral Specialists

Vicky
Vicky is a Cancer Rehabilitation and Clinical Exercise Specialist. She supports people with a cancer diagnosis and those managing long-term health conditions to be more active whilst building their strength and fitness.
With 15 years experience in the fitness industry, she is passionate about helping people to access and experience the benefits of physical activity. She runs 1-1 and group sessions alongside being a Physical Activity Referral Specialist.
After spending 10 years in Australia, she now lives in Buckinghamshire with her husband, two teenage children and their naughty Irish Terrier Rosie. She loves live music, crime novels and travelling.

Lavender
Lavender is a Doctoral researcher specialising in physical activity, specifically management and prevention of chronic diseases. She is a registered Senior Physical Activity and Health Practitioner, a Level 4 Cardiac Rehab Specialist Instructor and has a Masters degree in Exercise for Sports Injury and Health.
Lavender is an experienced exercise instructor with over 15 years of helping people become more active through tailored plans. She believes in this journey of life; small ailments are unavoidable. Living with them positively is the right way to get through, and everyone deserves to enjoy the benefits of exercise regardless of the conditions they face. Therefore, she excels in customising exercises to meet individual needs and functional goals.

Nicola
Nicola is a dedicated personal trainer and running coach with a passion for helping people feel strong, confident, and empowered through movement. She specialises in making fitness approachable, enjoyable, and sustainable for everyone, no matter their starting point.
Her approach prioritises safe, effective exercise that supports overall well-being, helping clients move in a way that feels good, fits their lifestyle, and keeps them motivated for the long run.

Kane
Kane is a Cancer Rehabilitation, Obesity & Diabetes, and Exercise Referral Specialist, whilst also running his own personal training business. Kane is committed to creating positive health changes across the community by supporting individuals with long-term conditions to lead more active, independent, and fulfilling lives. With over 5 years of experience in the exercise referral pathway, Kane is passionate about supporting people through safe yet effective physical activity and movement.
