Referral Pathway

Supporting you to move more, feel better, and stay independent!

Move Together Buckinghamshire is a free programme designed to help residents aged 50 and over, who are living with a long-term health condition, to be more active and improve their physical and mental wellbeing.

Funded by Buckinghamshire Council’s Public Health Team and delivered by Active in the Community (AITC), this three-year initiative is all about helping people move more in a way that suits them — with professional support every step of the way.

What Is Included?

If you’re eligible, you’ll receive up to 12 months of support, including:

  • A personalised 12-week physical activity plan, tailored to your needs and abilities
  • 1-2-1 guidance from a trained adviser
  • Help accessing local, affordable activity sessions
  • Ongoing encouragement to build confidence and stay motivated

Whether you’re just getting started or looking to return to exercise, we will help you find the right pace and activities for you.

Who Can Join?

The programme is available to residents who are either:
 
  • Registered with one of the 17 Deep End GP practices listed 

and/or

Eligibility Criteria

  • Over the age of 50 years old. 
  • Currently inactive or fairly inactive.
  • Motivated to increase activity levels to improve health and wellbeing.  
  • Living with a long-term health condition (see approved conditions listed)
  • Registered at one of the 17 listed GP practices or live in one of the Opportunity Bucks wards (mentioned above)

Exclusion Criteria:

    • Severe or poorly controlled asthma
    • Severe COPD
    • Currently accessing cardiac or cancer rehab or pulmonary services
    • Currently undergoing wider tests regarding the long term health condition(s)
    • Blood Pressure >180mmHg (systolic) or >100mmHg (diastolic)
    • Resting heart rate >100 bpm

Level 3 Referral Specialist

We are looking for an additional Physical Activity Referral Specialist Level 3 to be be part of this programme. The successful candidate will conduct initial assessments, plan, prescribe, implement and monitor a suitable exercise programme for individuals referred through the pathway.
